Individual
$90 per month
Adults, Ages 18-64
Plus a one time enrollment fee of $50
Couple
$170 per month
2 Adults, Ages 18-64
Plus a one time enrollment fee of $75; $25 enrollment if partner is currently enrolled.
Child
$45 per month
Ages 2-18
Plus a one time enrollment fee of $50; enrollment fee is waived if a parent is currently enrolled.
Visits for Children include all well visits and sick visits, school physical forms, sports physicals, camp physicals, etc. Please note that immunizations will be an additional fee which will be coordinated in advance.
Newborn/Baby Care
$90 per month
Age 0-24 months
Newborn package is from 0-2 years old. This includes: well visits at 2-3 days, 1, 2, 4, 6, 9, 12, 18 and 24 months, all routine developmental screenings, growth monitoring, counseling on breastfeeding/other feeding issues and any extra visits as needed. A vaccine package price will also be provided at the time of enrollment which will include all CDC recommended and school required vaccinations. I will also provide day care forms and immunization records as needed for childcare.
Plus an enrollment fee of $50; no enrollment fee if parents are currently enrolled.
College Student
$60 per month
Students enrolled in a degree or residency training program.
Plus a one time enrollment fee of $50
Additional fees
Additional fees will be discussed and agreed upon before service is administered.
Re-enrollment fee $250 per person (if you leave the practice and choose to re-enroll).
Form Completion fee (forms greater than 3 pages) fee based on time required.
Vaccines - in some instances, insurance can be billed for vaccines, or we can give your our discounted direct pricing.
